Providers
Talking about advance care planning isn’t always the easiest thing to do, but it is an important conversation to have with your patients. Voice Your Choice provides tools and resources to help start a discussion around advance care planning.
You can have a say in your care if you are unable to speak for yourself. Create an electronic advance care plan or upload an existing advance care plan to document your health care wishes and make sure they are easily accessible to loved ones and your health care providers.​
