Community partners are at the heart of the Voice Your Choice Program.

The Voice Your Choice program recognizes that community-based organizations serve as trusted leaders and are essential to providing information and education to their communities. Through trainings provided by Community Facilitators, the many communities that make up our region will have the opportunity to make their health care wishes known and accessible.

The Voice Your Choice program provides training, materials, support, and a small stipend to Community Facilitators. Please note that during the COVID-19 outbreak trainings are on hold.

Join us in sharing information and resources with our community about how to have health care wishes and values known in the event of a serious illness or emergency. Also share your knowledge and experience by providing input and feedback into the Voice Your Choice program and trainings. Contact us for additional information.

Become a Community Facilitator

Interested in becoming a Community Facilitator and sharing advance care planning information directly with the community? Community Facilitators attend a short class to learn about advance care planning and our tools and resources. No previous experience is needed, and training, materials, support, and a small stipend are provided to each Community Facilitator by the Voice Your Choice program.
